All children must have on file the following forms and they must be updated annually. The forms must be turned in at the time the child begins the program. Emergency information including the doctor's name and how parents may be reached is also needed for each child. Parents are asked to immediately update us on changes in telephone numbers, etc.

• Registration Form 2007-2008 School Year
  (1 page 8-1/2" x 14" document) or (2 page 8-1/2" x 11" document)
• Medical Form
• Authorization to Consent to Treatment of Minor
• Child Pick-Up Form
